Document Requirements for New Players
Players joining our travel teams are required to complete the US Club Soccer R002 Player Liability Waiver/Release and Consent Form. Please use the following link for the form.
A valid birth certificate is required. A clear photo in JPG or PNG format is acceptable.
A clear, shoulders‑up photo of the player taken in front of a neutral‑colored background.
Complete the Google Form at the link below. It includes file‑upload options for each required item.
Uniforms
Uniforms can be purchased directly from our vendor using the link below. The base package includes two jerseys, shorts, and a pair of socks, and all orders are shipped straight to your home.
Please confirm with your coach which jersey number your player should use to avoid any duplicates on the team.
When selecting sizes, please note that both youth and adult options are listed. The uniforms generally run true to size, but many families choose to order one size up to allow room to grow. Most players are able to get several seasons out of their uniform.
